Vitamin D is not just a bone vitamin. It acts like a hormone + local gene-regulating signal that affects:
*Bone and calcium
*Muscle strength and balance
*Immune function (infections, autoimmunity)
*Metabolic health (diabetes, obesity, cardiovascular disease)
*Pregnancy and infant health

1. Endocrine system
Kidney converts vitamin D to calcitriol (the “active hormone”).
Main job: keep calcium and bone normal.
2. Intracrine / paracrine / genomic system
Immune cells, placenta, and other tissues convert vitamin D inside the cell.
This is how vitamin D influences genes, inflammation, immunity, pregnancy, etc.

15 ng/mL – Bare minimum for calcium metabolism (prevent rickets/osteomalacia).
20 ng/mL – Enough for basic musculoskeletal function: balance, falls, fractures.
30 ng/mL – Better for:
Osteoporosis, fractures
Inflammatory bone disease
Secondary hyperparathyroidism
40+ ng/mL – Linked to lower risk of:
Metabolic disorders (obesity, diabetes)

50–80 ng/mL – The paper argues this range may be ideal for:

Lower risk/severity of infections, sepsis, some cancers
80 ng/mL – Proposed in the paper for certain “vitamin D resistance” states (e.g., some autoimmune or inflammatory conditions), but this is more experimental and definitely specialist-level.

Pregnancy & Breastfeeding
RCTs using 4000 IU/day in pregnancy improved outcomes and were safe.
Maternal levels 40 ng/mL linked to fewer complications and better birth outcomes.
To reach that, the paper suggests 4000–6000 IU/day in pregnancy.
