Eli Lilly announced that it is lowering the cash prices of single-dose vials of its blockbuster weight-loss drug Zepbound on its direct-to-consumer platform, LillyDirect—marking a significant move to expand access to GLP-1 medications as demand soars across the U.S. ()
The new pricing structure allows cash-paying patients with a valid prescription to purchase Zepbound vials for $299 to $449 per month, depending on the dose. Previously, prices ranged from $349 to $499 per month. The cuts took effect December 1.
The price drop arrives just weeks after President Donald Trump finalized agreements with Eli Lilly and rival manufacturer Novo Nordisk aimed at making GLP-1 drugs more affordable. Those deals will introduce Medicare coverage for certain obesity treatments for the first time and launch a federal direct-to-consumer website, TrumpRx, early next year.

Prices for Zepbound Drop as Rival Novo Nordisk Also Cuts Costs
Under the new pricing:
- Starting dose: $299 per month (down from $349)
- 5 mg dose: $399 per month
- Higher doses: $449 per month (down from $499)
Zepbound’s list price is approximately $1,086 per month, and insurance coverage for weight-loss medications remains inconsistent in the U.S., leaving many patients to pay out of pocket. Lower prices through LillyDirect are expected to ease access barriers, particularly for patients without insurance coverage.
Novo Nordisk recently reduced prices on its own obesity and diabetes drugs — Wegovy and Ozempic — offering existing cash-paying patients a lower monthly rate of $349. It also introduced a temporary $199 introductory price for new patients on the two lowest doses.
Move Comes Ahead of Trump-Lilly Agreement on Multi-Dose Pen
While the Trump administration’s agreement with Eli Lilly focuses on lowering prices for a future multi-dose pen version of Zepbound — pending FDA approval — Monday’s announcement pertains to the existing single-dose vials, allowing more patients to benefit immediately.
Eli Lilly said the goal is to broaden options for patients and streamline access as demand grows. “We will keep working to provide more options — expanding choices for delivery devices and creating new pathways for access — so more people can get the medicines they need,” said Ilya Yuffa, president of Lilly USA and global customer capabilities.
Lilly Stock Slips Despite Long-Term Growth Momentum
Shares of Eli Lilly, which have surged more than 36% this year, fell nearly 2% following the announcement. Despite price reductions potentially lowering revenue per dose, Lilly’s overall sales have continued to climb due to skyrocketing demand for Zepbound and its diabetes drug Mounjaro.
The company recently became the first pharmaceutical company to surpass a $1 trillion market value, highlighting its dominance in the GLP-1 obesity drug market.
Adoption of Single-Dose Vials Remains Unclear
The single-dose vial format, introduced in August 2024, requires patients to use a syringe and needle to administer each injection — a process some may find less convenient than auto-injector pens.
Eli Lilly has not disclosed how many patients currently use the vial format, though it recently reported that more than one-third of new Zepbound prescriptions come through its direct-to-consumer channel.
Meanwhile, Novo Nordisk continues to respond to heavy demand by lowering cash prices and offering limited-time incentives for new patients seeking to start Wegovy or Ozempic.
