When a patient is on hospice, they receive ongoing evaluations from the hospice team to determine if they continue to be eligible for hospice. Every patient has to meet certain hospice criteria. When it comes to any form of dementia, (ie: Vascular Dementia, Alzheimer’s Disease, Frontotemporal Dementia, Lewy Body Dementia, Parkinson’s disease, etc), it’s not uncommon for the person’s health and disease process to plateau, or even have ups and downs where to decline isn’t obvious. This video is all about tips to improving assessment skills and communication with the hospice team to advocate for the person with dementia to remain on hospice.
